Contribution of hypertension to the formation of endpoints of diseases of the circulatory system according to population studies

Circulatory system diseases occupy the first place in the mortality structure in Ukraine (63,7%), the endpoints generally accepted in clinical and epidemiological studies in most cases are formed due to CAD and its risk factors. The paper presents approaches to assessing the contribution of arterial hypertension (AH) to the formation of endpoints of cardiovascular disease. Within 5 years, organized contingents of men (1868) and women (2146) 20—59 years old and an unorganized population of men 40—59 years old (7629), residents of the administrative district of Kharkov were examined. Endpoints included new cases of fatal and non-fatal myocardial infarction (MI). In the presence of CAD, the number of MI was 4.5 times greater than in the absence of it, to a greater extent this concerned MI with a fatal outcome, where the ratio was seven fold — 7.3 versus 1,1 per 1000 people / years. The prevalence of hypertension among people aged 20—59 years of both sexes was 23,8%. In patients with AH, the number of new cases of MI was 70% higher than in its absence, but there were three times more lethal MI (3,6 versus 1.3 per 1000 people / years). The spectral function of the magnitude (P, x) of the distribution of mortality from myocardial infarction was calculated. In hypertension, this indicator was 0,39, lower for coronary heart disease (P = 0,9), but significantly higher for overweight (P = 0,18) and low physical activity (P = 0,08). With a combination of all three risk factors, a synergistic effect is observed, which, however, is lower than for CAD.
