Biliary pathology is the most common cause of the development of acute and exacerbation of chronic pancreatitis. A feature of biliary pancreatitis, which distinguishes it from other forms of pancreatitis, is the combination of exocrine pancreatic insufficiency with chronic biliary insufficiency, which affects the quality of the digestive process. The main development factors, symptoms, diagnostic algorithms for verifying the diagnosis of biliary pancreatitis are presented. The article discusses the main directions in the treatment of biliary pancreatitis, taking into account the elimination of the causes of the disease — choledocholithiasis, strictures of the terminal part of the common bile duct and duodenal papilla, dyskinesia of the gallbladder and sphincter of Oddi. For this, the modern possibilities of conservative, endoscopic and surgical treatment of this disease are used.

Key Words: biliary pancreatitis, exocrine pancreatic insufficiency, 13C-triglyceride breath test, biliary pathology, pathogenetic therapy.
